First it pulls her legs apart until her ankles reach the ends of the iron spreader then she is slowly hoisted up towards the ceiling by her ankles, as her crotch is raised into the air the rope between her legs pulls tight across her crotch. Masie switch the winch on and off until her ankles are as high as she dare go.
